Prize includes one (1) Garmin nuvi 40 GPS and office cleaning from CMS of Madison. Cleaning valid for a maximum of 20x20 square feet of office space. Office must be located within Dane County and be cleaned between the hours of 8:00am and 4:00pm Monday-Friday. The winner of the contest must be present during the awarded cleaning and surfaces must be cleared prior to arrival. CMS is not responsible for broken items. Cleaning includes detailed dusting, vacuuming, sweeping and dusting of floors, and taking trash.
You must be 18 years of age or older at time of entry to be eligible for this contest. This contest is void where prohibited by law.

If a winner is unreachable after seven (7) days, or if that winner is unavailable for prize fulfillment, an alternate winner will be selected. If Channel3000.com cannot find an eligible winner for the prize, that prize will not be awarded.

Each winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received.
This contest is not intended for gambling. If it is determined that a contestant is using the contest for gambling purposes they will be disqualified and reported to authorities.
