Winner will be notified by email, mail or phone during the week following the Drawing Date. If a winner is unreachable after seven (7) days, or if that winner is unavailable for prize fulfillment, an alternate winner will be selected. If Channel3000.com cannot find an eligible winner for the prize, that prize will not be awarded.

Prize includes (1) three month membership to Perennial Yoga and Fitness, worth $297. Membership will run three consecutive months from the time of the first class, including unlimited yoga and meditation. All participants receive one (1) free class at Perennial Yoga and Meditation.

One winner will be drawn on Monday, October 29, 2012. Entry deadline is Sunday, October 28, 2012 at 11:59 PM.
The decisions of the Contest judges regarding entries and winner shall be final.

This Contest is open to legal residents of Wisconsin, Illinois, Iowa, and Minnesota who are 18 years of age or older. The Contest is subject to all applicable federal, state and local laws and regulations. This offer is void where prohibited or restricted. Employees of Television Wisconsin, Inc., Perennial Yoga and Meditation, and any Participating Companies, the advertising and promotion agencies representing any of such companies, and members of those employees' families are not eligible to enter or win.
By submitting an entry, each entrant agrees to all the terms and conditions of these rules; agrees to be bound by these rules and the decisions of the Contest judges; and consents to the use of his/her name, likeness and other information for publicity purposes. Each winner accepting a prize and each guest traveling with and/or accompanying the winner consents to the use of his/her name and/or likeness for advertising, trade and promotional purposes without further compensation, except where prohibited by law. Each entrant waives the right to assert, as a cost of winning any prize, any and all costs of verification and redemption or travel to redeem a prize and any liability that might arise from redeeming or seeking to redeem a prize. Winner is responsible for taxes and title fees. Each entrant releases each of the following from any and all liability with respect to or arising out of the acceptance, use, misuse or possession of any prize and/or the loss or misdirection of any prize: Television Wisconsin, Inc., Sponsor and the Participating Company; their respective affiliates, subsidiaries, divisions, franchises, advertising and promotional agencies; and their respective officers, directors and employees. Television Wisconsin, Inc. reserves the right to cancel, amend and/or revoke this Contest due to printing errors or other mistakes.
Television Wisconsin Inc. is not responsible for any problems/failures related to any contestant’s lost, stolen or misdirected entry. Incomplete entries will be disqualified.
Odds of winning depend on the number of eligible entries received.
Automated voting of any kind is not allowed. Detection of said automated voting will lead to votes being voided.
Only one registered account per voter. If multiple accounts are detected for a single voter, they will be disabled and votes disqualified.
Channel3000.com reserves the right to substitute any prize with another prize of equal or greater value.
Channel3000.com reserves the right to restrict the local winners to only persons whose primary residence is within a 60 mile radius of the affiliate's' office.
Employees of Channel3000.com, its respective parent, affiliates, subsidiaries, advertising sponsors and promotional agencies, and the immediate family members of each are not eligible.
Contest entrants agree to abide by the terms of these Official Rules and by the decisions of the contest commissioners, which are final on all matters pertaining to the contest.

Each winner, by acceptance of the prize, agrees to release all Sponsors, and their parent and subsidiary companies, their officers, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the receipt, ownership, or use of the prize.
UPICKEM and Channel3000.com are not responsible for Internet crashes or slowdowns caused by network congestion, viruses, sabotage, satellite failures, phone line failures, electrical outages, natural disasters or acts of man or God.
All results posted are unofficial until winners respond to e-mail notification.
Each winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received.
This contest is not intended for gambling. If it is determined that a contestant is using the contest for gambling purposes they will be disqualified and reported to authorities.
