To enter, complete an official registration form located on Channel3000.com or DairyDoingMore.org during the nomination period ending 11:59 p.m. Thursday, May 16, 2013. Entry must include an explanation on why the nominee deserves to be considered for the “Salute to Wisconsin Dairy Professionals” award.
Nominees and nominators must be 18 years of age or older at time of entry to be eligible for this contest and a Wisconsin resident residing in one of the following counties: Adams, Columbia, Crawford, Dane, Dodge, Grant, Green Lake, Green, Iowa, Jefferson, Juneau, Lafayette, Marquette, Richland, Rock, Sauk, Walworth or Waukesha. This contest is void where prohibited by law.
One entry per person will be eligible for judging. Nominator can nominate one’s self.
Entries will be judged by a panel of experts. Judging criteria includes, but not is limited to the following: Contributions to the dairy community and the community in which they live. Nominees should be a part of, and have an impact on the dairy industry (i.e. dairy farmer and/or family, cheese maker, educator, government, etc.)
Eight (8) winners will be selected from all approved entries that are received. Winners will be notified by mail, email, or phone call between May 17th, 2013 and May 22nd, 2013. If a winner is unreachable after seven (7) days, or if that winner is unavailable for prize fulfillment, an alternate winner will be selected. If Channel3000.com cannot find an eligible winner for the prize, that prize will not be awarded. One (1) of the eight (8) winners will be saluted per week from June though July.
Winners will be posted on Channel3000.com, DairyDoingMore.org, and broadcast on WISC-TV3 and TVW. Winners will also be awarded a plaque. Plaque cannot be exchanged or redeemed for cash.
As a condition of entry, winner agrees to allow Morgan Murphy Media, the Wisconsin Milk Marketing Board and participating sponsors the use of his/her name, and/or likeness and video/photo for publicity purposes.
All photos submitted with a nomination become the property of Channel3000.com and
absolutely no photos will be returned. Copyrighted photos must be accompanied
by photographer’s release. Channel3000.com reserves the rights to refuse any
photograph it determines to be unacceptable.
By submitting an entry, each entrant agrees to all terms and conditions of these rules; agrees to be bound by these rules and the decision of the contest judges; Each entrant agrees to release and hold harmless Morgan Murphy Media, the Wisconsin Milk Marketing Board, and any and all sponsors and persons affiliated with the contest from any liabilities and all claims of damage. Morgan Murphy Media reserves the right to cancel, amend and/or revoke this contest due to printing errors or other mistakes. Winner can be responsible for taxes and title fees.
Odds of winning depend on the number of eligible entries received.
Automated voting of any kind is not allowed. Detection of said automated voting will lead to votes being voided.
Only one registered account per voter. If multiple accounts are detected for a single voter, they can be disabled and votes disqualified.
Channel3000.com reserves the right to substitute any prize with another prize of equal or greater value.
Channel3000.com reserves the right to restrict the local winners to only persons whose primary residence is within a 60 mile radius of the affiliate's' office.
Employees of Channel3000.com, Wisconsin Milk Marketing Board, and their respective parents, affiliates, subsidiaries, advertising sponsors and promotional agencies, and the immediate family members of each are not eligible.
Contest entrants agree to abide by the terms of these Official Rules and by the decisions of the contest commissioners, which are final on all matters pertaining to the contest.
Entrants further grant to Channel3000.com the right to use and publish their proper name and state online and in print, or any other media, in connection with the Contest.
Each entrant waives the right to assert or claim any cost of winning the award, and any liability that might arise from redeeming or seeking to redeem a prize (i.e. travel for production of TV commercial, etc).
Channel3000.com reserves the right to use any and all information related to the Contest, including information on contestants obtained through the contest, for marketing purposes or any other purpose, unless prohibited by law.
The Contest and all of the related pages, contents and code are copyright, UPICKEM, and/or Channel3000.com. Copying or unauthorized use of any copyrighted materials, trademarks or any other intellectual property without the express written consent of its owner is strictly prohibited.
Each winner, by acceptance of the prize, agrees to release all Sponsors, and their parent and subsidiary companies, their officers, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the receipt, ownership, or use of the prize.
UPICKEM, Wisconsin Milk Marketing Board, and Channel3000.com are not responsible for Internet crashes or slowdowns caused by network congestion, viruses, sabotage, satellite failures, phone line failures, electrical outages, natural disasters or acts of man.
All results posted are unofficial until winners respond to e-mail notification.
Each winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received.
This contest is not intended for gambling. If it is determined that a contestant is using the contest for gambling purposes they will be disqualified and reported to authorities.
Television Wisconsin, Inc. reserves the right to
cancel, amend and/or revoke this Contest due to printing errors or other
mistakes.
Television
Wisconsin Inc. is not responsible for any problems/failures related to any
contestant’s lost, stolen or misdirected entry. Incomplete entries will be
disqualified.
Channel3000.com reserves the right to change the rules at any time.
