There will one (1) grand prize winner with a $1,000 cash prize and one (1) runner up winner with a $500 cash prize each. 

You must be 18 years of age or older at time of entry to be eligible for this contest. This contest is void where prohibited by law.

Both winners will be selected by a panel of judges after Monday, September 9th, 2013 and no later than Monday, September 30th, 2013. Entry deadline is Sunday, August 25th, 2013 at 11:59 PM. 

Qualified video entries can be no more than three minutes in length maximum.  All video entries must be appropriate for broadcast.  As a result, any entries with inappropriate language or content will be rejected. All video entries shall focus on the importance of early detection and screening.   All video entries shall refrain from using specifics of individual medical histories or any individual health information

The decisions of the Contest judges regarding entries and winners shall be final.

Prizes may not be substituted and are not refundable. Odds of winning will depend on the number of eligible entries received. 

Winners will be notified by email, mail or phone during the week following the selection date. If a winner is unreachable after seven (7) days, or if that winner is unavailable for prize fulfillment, an alternate winner will be selected. If Channel3000.com cannot find an eligible winner for the prize, that prize will not be awarded. 

Channel3000.com reserves the right to substitute any prize with another prize of equal or greater value. 

This Contest is open to legal residents of Wisconsin, Illinois, Iowa, and Minnesota who are 18 years of age or older. The participation of youth is encouraged with the consent and supervision of an adult 18+. The submission of video must be entered by an adult. The Contest is subject to all applicable federal, state and local laws and regulations. This offer is void where prohibited or restricted. Employees of Television Wisconsin, Inc., Turville Bay MRI & Radiation Oncology Center, and any Participating Companies, the advertising and promotion agencies representing any of such companies and members of those employees' families are not eligible to enter or win. 

By submitting an entry, each entrant agrees to all the terms and conditions of these rules; agrees to be bound by these rules and the decisions of the Contest judges; and consent to the use of his/her name, the video entry, likeness and other information for publicity purposes. By submitting a video for consideration, the creator releases all right to video and its content for promotional use by Television Wisconsin, Inc., Turville Bay MRI & Radiation Oncology Center, and any Participating Companies. 

Each winner accepting a prize and each guest traveling with and/or accompanying the winner consent to the use of his/her name and/or likeness for advertising, trade and promotional purposes without further compensation, except where prohibited by law. Each entrant waives the right to assert, as a cost of winning any prize, any and all costs of verification and redemption or travel to redeem a prize and any liability that might arise from redeeming or seeking to redeem a prize. Winner is responsible for taxes and title fees. Each entrant releases each of the following from any and all liability with respect to or arising out of the acceptance, use, misuse or possession of any prize and/or the loss or misdirection of any prize: Television Wisconsin, Inc., Sponsor and the Participating Company; their respective affiliates, subsidiaries, divisions, franchises, advertising and promotional agencies; and their respective officers, directors and employees. Television Wisconsin, Inc. reserves the right to cancel, amend and/or revoke this Contest due to printing errors or other mistakes. 

Employees of Channel3000.com, its respective parent, Turville Bay MRI & Radiation Oncology Center, affiliates, subsidiaries, advertising sponsors and promotional agencies, and the immediate family members of each are not eligible. 

Contest entrants agree to abide by the terms of these Official Rules and by the decisions of the contest commissioners, which are final on all matters pertaining to the contest. 

Entrants further grant to Television Wisconsin, Turville Bay MRI & Radiation Oncology Center, and any Participating Companies the right to use and publish their proper name and state online and in print, or any other media, in connection with the Contest.

Channel3000.com reserves the right to use any and all information related to the Contest, including information on contestants obtained through the contest, for marketing purposes or any other purpose, unless prohibited by law.

Each winner, by acceptance of the prize, agrees to release all Sponsors, and their parent and subsidiary companies, their officers, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the receipt, ownership, or use of the prize.

UPICKEM and Channel3000.com are not responsible for Internet crashes or slowdowns caused by network congestion, viruses, sabotage, satellite failures, phone line failures, electrical outages, natural disasters or acts of man or God.

All results posted are unofficial until winners respond and verify notification.

Each winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received.

This contest is not intended for gambling. If it is determined that a contestant is using the contest for gambling purposes they will be disqualified and reported to authorities.

Television Wisconsin Inc. is not responsible for any problems/failures related to any contestant’s lost, stolen or misdirected entry. Incomplete entries will be disqualified.
